    To be eligible to vote in the June 5, 2012, State Primary Election, you must register to vote by
    May 21, 2012

    To vote in the next election, you must mail or deliver this form to your county elections office

    Any registered voter may vote using a vote-by-mail ballot instead of going to the polls on Election Day. The form must be returned at least 7 days before the election
